Depends on what I'm doing.

Math Notes: 10/11
French: Le 11 octobre 2004
Homework and stuff that requires a date: 10/11/04 or October 11, 2004
Everything else: Date? What date? :P

Either:

12th October 04
Tuesday 12th October 04

or

Mardi le douze octobre (if it's in French)

I write it the way I was tought in school years and years ago, the classic way.

Full - October 12, 2004 (Sometimes) October 12th, 2004
Short - 10/12/04

Thats the classic way. :D

It depends, on basic homework, I use 10/12/04 (I've always had a problem with DD/MM/YY, probably because I was taught otherwise, and it makes more sense for me to put it the way I would say it *shrugs*.)

When I'm putting dates on my essays for writing class I put:
October 12, 2004 (Yeah, the basic)

I write it DD/MM/YY - it's the way I grew up learning.

Gone, forever.

Always in the form of MM-DD-YY

I use both dashes (MM-DD-YY) and slashes (MM/DD/YY) most of the time. The only time I actually write the date out, like others have said in this thread, is on reports or formal stuff.

10-14-04. And when I write it out it's 14 October 2004. Mix and match. Or when I'm feeling annoying, El 14 de octubre, 2004.

I write in both formats.

10/14/04 or 14/10/04 and Oct 14, 2004 or 14 Oct 2004.

I use the dd/mm/yy format for Spanish class or when I randomly feel like it ^^;
