Is this getting closer to what you were thinking? I couldn't get the slippers to "hang" and couldn't find quite what I wanted in a search. But I did use the mirror (touched up of course) and the second mask without the top. I put the subtext on the sig just to give an idea, nothing on the avi. If this is acceptable image wise, let me know what you want written on the sig/avi. I'll do my best 
(which I get the feeling isn't very good with this theme)
Edit, found some ballet type slippers.
Have the sig saved without the text, so if you want something different it should work in that space. Is this closer to what you wanted?

This was actually my friend's set (I made it for her, obviously), but I changed the text placing, the avatar image, and obviously the name. Same subtext and signature picture though. Hope you don't mind, I had to remake it any way.
